Collin Blanchard serves as SALTA’s Director of Population Health Management (PHM), where he leads efforts to translate data into meaningful action. By designing and overseeing targeted interventions, he helps improve outcomes, manage chronic disease, and reduce avoidable costs ensuring SALTA’s care model remains focused, measurable, and mission-driven.
Before joining SALTA, Collin worked at General Motors in manufacturing operations and supply chain risk management.
